Process for Submitting Student Medical Forms

We have updated our processes to facilitate you better.

 Prospective Students for Academic Year 2021/22 do not have to visit the Medical Centre to submit medicals that were completed at health facilities external to the Campus.

 To process this submission the student is required to:

  1. Fully complete Part 1 of the Student Medical Form.
  2. Schedule an appointment for your medical doctor/clinic to fully complete Parts 2 & 3 of the Medical Form. Ensure that the Form is signed and stamped by the medical doctor/clinic.
  3. Obtain a copy of the result of the blood test and any other diagnostic investigation as deemed appropriate by the medical doctor.
  4. Have your Immunization Card assessed to determine if the vaccine requirements have been met.
  5. Obtain booster dose(s) of vaccine(s) to meet requirements.

Receipt of the electronic mail will be acknowledged within 5 working days.


Once this process has been correctly completed, your name will be submitted to the Safety & Security Department for the processing of your Student Identification card.
